Prescription Drug Program Formulary
The Prescription Drug Program Formulary is designed to include all therapeutic categories and provide physicians with prescribing options. When your provider prescribes a formulary medication, you pay a lower copayment for the drug. Prescribed medication that is not part of the formulary means a higher out-of pocket cost for you. It is therefore very important that you work with your doctor to ensure that your prescribed medication is on the formulary, whenever possible.
If you are enrolled in a formulary program that does not require pre-authorization, please refer to this Formulary Guide.
If you are enrolled in a formulary program that requires prior authorization for certain medications, please refer to this Formulary Guide.
You can view our procedures that support safe prescribing to see a list of prescription drugs that need prior authorization.
